Please try to use this form as it makes requests much easier and faster to fill from our icon makers.  You do not have to have a photo in your icon request, if you would prefer just text, please just put n/a on the photo html section!

Once your icon has been completed, it will be placed as a comment to your request with crediting information.  When you have picked up the icon and saved it to your own computer, please post a comment so that the request and thread may then be deleted.  Thank you, and enjoy!
